Gastroenterology

Objective

  • Competency in diagnosis and management of common gastroenterology and hepatology conditions
  • Competency in managing childhood nutrition and growth
  • Competency in handling gastroenterology and hepatology emergencies
  • Competency in performing and interpreting pH/impedence studies
  • Advanced trainees are expected to be able to perform procedures like endoscopy, liver and rectal biopsies
  • Research is a strongly encouraged component of training

Course Contents

Teaching Programme

  • Assessment, investigation and management of Paediatric patients with gastrointestinal and liver disorders (eg. acute and chronic diarrhoea, inflammatory bowel disease, gastrointestinal hemorrhage, obstructive jaundice in infancy, liver failure, chronic hepatitis B)
  • Upper GI endoscopy and colonoscopy, sclerotherapy of varices and polypectomy.
  • Small intestinal biopsy with Watson Biopsy Capsule, partial-thickness rectal biopsy and percutaneuos liver biopsy.
  • Urea breath test
  • 24-hour oesophageal pH/impedence monitoring
  • Review of biopsy slides, X-ray studies and isotope scans with the respective specialist on a regular basis.
  • Principle and practice of enteral and parenteral nutrition.

Method

Method of Training

  • Daily inpatient ward rounds and case discussion with consultant
  • Consultant supervised outpatient clinic (twice a week)
  • Consultant supervised liver clinic (monthly)
  • Combined multi discipline journal club with paramedical staff (monthly)
  • Performing and interpreting 24-hour pH/impedence studies
  • Hands-on procedure training for endoscopy, liver and rectal biopsy
  • “Hot-topic” management and work flow discussion
  • Transition observership programme for inflammatory bowel disease and hepatitis B (in conjunction with adult Gastroenterology and Hepatology Department of
  • Singapore General Hospital)
  • Systemic lectures, tutorials and ward rounds
  • Seminars, journal reading and research projects
  • Laboratory experience through bench work and laboratory based research projects. Opportunities to collaborate in biomedical research with NUS, Singapore-MIT alliance and A*STAR can be arranged.

Pre-requisite

All applicants must fulfil the requirements for temporary registration with the Singapore Medical Council.

Two-year posting

First Year

  • Emphasis on clinical training in general Paediatric Gastroenterology, Hepatology and Nutrition at inpatient and outpatient level.
  • Familarize with radiological and radioisotope studies and laboratory tests
  • Hands-on experience in upper gastrointestinal endoscopy, small intestinal biopsy, rectal biopsy and percutaneous liver biopsy and continuous oesophageal pH/impedence monitoring
  • Active involvement in nutritional support, both enteral and parenteral, of paediatric patients.

Second Year

  • Increasing responsibility in the management of Paediatric gastroentrology patients
  • Commence training in colonoscopy, polypectomy and sclerotherapy of varices
  • Increasing labarotory involvement including histopathology, immunology and research

Certificate of fellowship at the end of the attachment

Positions/Training Places

One to two Fellowship training positions will be given for a given duration
