The Sleep Disorders service is part of the Respiratory Medicine Service and has a 2-bedded and 8-bedded facility for paediatrics and adults respectively. The paediatric service sees children from newborn to 18 years old and the adult sleep service sees adolescents and adults. A wide range of disorders are seen including obstructive sleep apnea hypopnea syndrome, nocturnal alveolar hypoventilation, parasomnias, sleep association disorders, periodic limb movement disorders, Narcolephsy, insomnia, REM sleep behaviour disorders and other non-respiratory related sleep disorders.

The programme provides the opportunity for

  • Exposure to both adult and paediatric sleep disorders including non respiratory related disorders in the inpatient and ambulatory setting
  • Exposure to a multi-disciplinary approach to management of these disorders including involvement  of the neurologist, ENT surgeon, psychiatrist and psychologist.
  • Learning to ‘hook up’, scoring and interpreting the adult and paediatric sleep study
  • Exposure to general Paediatric respiratory disorders

Description
6-month posting
  • Conduct 2 – 3 sessions per week of the ambulatory clinics consisting of the general respiratory referral clinics, high risk asthma clinic, bronchopulmonary dysplasia and the sleep clinic, under the supervision of a respiratory consultant
  • Rostered to run the respiratory / sleep consult service
  • 1 month rotation to the pulmonary function / Sleep laboratory
  • Observe or assist in an overnight polysomnogram
  • 6 weeks rotation to adult sleep service (SGH)
  • Programme  can be incorporated into a 1-year respiratory / Sleep fellowship
3-month posting
  • 10 weeks paediatric and 2 weeks adult sleep service


Expected learning / learning objectives

For one-year posting (Paediatric Respiratory and Sleep)

Upon completion, the trainee should be able to manage acute and chronic respiratory problems with some confidence. He should gain an understanding of pulmonary function tests and their interpretation as well as observe or performed flexible bronchoscopies and an understanding of polysomnography and sleep-related disorders.

 

For six-month posting,

Upon completion, the trainee should be able to manage sleep disorders in children, score and interpret polysomnograms as well as gain a basic  understanding and appreciation of adult sleep disorders.

 

For three-month posting,

Upon completion, the trainee should have a good  overview of sleep disorders and  be able to  manage common sleep disorders in children. He should also gain a basic appreciation and understanding of scoring and interpreingt sleep reports.

 

Teaching Programme

  • Didactic lectures
  • Observed / staffed inpatient and outpatient consults with the respiratory physicians, neurologists, otolaryngologists, psychiatrists, psychologists
  • Observer / hands-on with polysomogram scoring and reporting
  • Night rotations with exposure to observe and learn aspects of nocturnal polysomnograms, CPAP / BiPAP titration and oxygen titration
  • Opportunity for research exposure
  • Opportunity for observation of surgical interventions in the operating theatre
