Paediatric Subspecialty Training for Rheumatology
  • Qualification: MRCP or equivalent from country of origin
  • 3 - 12 moths attachment (sponsor required)
  • Outpatient clinics and inpatient consultations, combined clinic
  • Hands-on for intra-synovial injections
  • Appropriate medical insurance needed for co-management and procedures
  • Participation in local research or study leading to presentation and /or publication
  • On completion: Certification of training in specialised area
 
Paediatric Rheumatology Service
  • One paediatric consultant
  • Annual number of new cases: approximately 70 new cases per year
  • Scope of patients seen:
    • SLE ( systemic lupus erythematosus)
    • JDM ( juvenile dermatomyositis)
    • JIA (juvenile idiopathic arthritis)
    • Vasculitis
    • Reactive arthritis
    • Septic arthritis
    • Limb aches and joint pains for investigation
    • Suspected immunological /autoimmune disease
  • Linked closely to immunology / allergy service in KKH (Rheumatology / Immunology / Allergy service)
  • Affiliation to Tan Tock Seng Hospital adult rheumatology, immunology, allergy unit (RIA)
 
Services Provided
  • Outpatient clinics 3 x/week ( Mon, Tues, Wed) about 8 - 12 patients per session
  • Inpatient referrals, consultations, admissions
  • Combined clinic with orthopaedics, rehabilitation service
  • Close link with referrals between orthopaedic, renal , endocrine, eye, dermatology services
  • Intra-synovial joint injections ( in and outpatient basis, with and without sedation) for joints, tendon sheaths etc.
  • Juvenile Arthritis Club, Juvenile Lupus Club, affiliated to National Arthritis Club

Additional Planned Projects
  • Bisphosphonates in osteoporosis in children with CT disease
  • NSAID hypersensitivity reaction in children - joint project with immunology service (KKH), European Network of Drug Allergies and TTSH RIA dept
  • Vaccines: attenuated live vaccines for children on chronic steroid therapy - joint project with immunology and infectious diseases services, KKH
  • Complementary and alternative medicine in children with rheumatic disease
 
Teaching
  • Informal rheumatology bedside and clinic teaching with hands on experience for fellow
  • Formal lectures / teaching on rheumatology - currently incorporated into departmental MO bedside tutorials, MO trainee core lecture series, exam slide quiz, orientation lecture, department journal club
  • Paedatric department teaching- morbidity, mortality and CICU rounds, journal club, monthly invited speakers, X-ray round, hospital conference
  • Teaching at GP updates, public forums, parents and children of JAC and JLC at club activities
 
Fellow will be expected to
  • Observe and co-manage outpatient cases
  • Co-see and co-manage inpatient referrals
  • Learn intra-synovial aspiration and injection
  • Participate in guided journal club, teaching sessions
  • Participate in local research, study, and present findings, publish as appropriate
  • Observe physiotherapy session in gym, hydrotherapy , making of splints and orthotics
  • Participate in combined clinic
  • Attend uveitis clinic, renal, eye, dermatology clinic as appropriate
  • Attend local lectures or programmes in rheumatology conducted by RIA dept in KKH / TTSH or visiting lecturers
  • Participate in some activities of support groups for children (juvenile arthritis club, juvenile lupus club)
  • Attend Workshops in Rheumatology (yearly practical rheumatology meeting organised by Singapore and Malaysian Rheumatology Associations - SSIAR, MSR) usually in July / August.
