- Has no referral from a doctor (i.e. self-refer) or
- Is referred by a private practitioner or hospital or
- Is referred by a restructured hospital where you are a private patient or

* Subsidised referral applies when a patient: |
- Is a Singaporean or Singapore PR and referred by a polyclinic or
- Is referred by a restructured hospital where the patient is a subsidised patient or
- Is discharged from inpatient B2 or C Ward Class

| For patients who have not seen their doctors for more than 12 months: |
- A Private First Consultation fee would be imposed at your next visit or
- A new referral from the polyclinic would be required should you wish to continue your care in KKH as a subsidised patient
