KK Women’s and Children’s Hospital Prescription Refill Courier Service

KK patients can now obtain your prescribed medication without having to make a trip to the Hospital again.

This service is only available for registered patients of KK Women’s and Children’s Hospital staying in Singapore.

What should I do if I want my medication to be delivered to me?

If you are at the Pharmacy, leave your prescription with the Pharmacy Staff. Our staff will arrange with you on the preferred date of courier. Charges for the medications and courier service can be paid at cashiers at the Pharmacy.

You can also download the attached form, fill up the details and fax the completed form together with your prescription to the Pharmacy at +65 6394 1504

Once Pharmacy receives the fax copy of the form and prescription, we will contact you to confirm the details of the courier.

What are the charges for the courier service?

It is S$10.00 for same day courier of medications/items.

You may contact our Pharmacy for more information on the charges for the courier service.

The courier service charge is payable to KK Women’s and Children’s Hospital and does not include the cost of medications. Prices are subjected to change.

How can I pay for my medication?

  • Cash – the exact amount is to be sealed in an envelope and handed to the courier service personnel
  • Cheque to be crossed and made payable to “KK Women’s and Children’s Hospital. The cheque is to be sealed on an envelope and handed to the courier service personnel.
  • AXS stations, SAM machines and Post Offices
  • Online payment using VISA or Mastercard through our webpage at www.kkh.com.sg

Can I request for other retail items to be delivered to me?

You may request for other retail items to be delivered to you, please contact our Pharmacy staff at +65 6394 1500/1501 for further information.

Other Important Information

  • Once an order is confirmed, no changes can be made upon delivery
  • Medications/Items sold are non exchangeable and non refundable
  • Please ensure that someone is at the specified address to receive the medications/items at the agreed date/time.
  • Please check that the delivered items are correct and in good condition.

Request for Medical Reports

Here in KKH, medical records are kept strictly confidential. Access to your medical records can only be released with your written consent, unless required for police investigation. The hospital's Department of Document Management Services provides this service.

Who can request for a medical report?

  • Patient/patient's parent if patient is below 21 years of age.
  • Patient’s next of kin (if patient is a deceased, a copy of the death certificate and other documents e.g. marriage certificate, birth certificate, letters of administration as proof of your relationship to the deceased must be submitted together with the application).
  • Interested third parties with the consent of the patient.

How do I apply for a medical report?

By mail

  • Please complete the Consent for Release of Medical Information Form to make the request. All sections in the form must be fully completed and duly signed.
  • For insurance claim, please enclose the insurance claim form with the authorization section of the claim form fully completed and duly signed.
  • Please attach a photocopy of your NRIC (front and back view) / identification documents and enclose a crossed cheque made payable to "KK Women’s and Children’s Hospital Pte Ltd".

How will I receive the completed medical report / insurance form / copies of medical records?

Patients can request for the medical report / insurance form / copies of medical records to be:

  • Mailed to them – Mailing address to be provided at the point of application. (a fee of $2.50 for registered mail is applicable)
  • Collected personally – Patient’s contact number to be provided at the point of application. (patient will be contacted once the report is ready for collection)
  • Collected by a third party – Authorisation letter is required if a third party is collecting on patient’s behalf.

Why do I need to pay for a medical report?

The regular consultation and hospitalization fees paid do not include cost for a medical report as not every patient requires one. A fee has to be levied for this additional service of providing medical reports to cover the hospital’s administrative costs and the doctor’s professional input. This is a standard practice across all the Restructured Hospitals.

How much do I need to pay and how soon can I obtain the requested medical information?

As a general guide, the charges and time frame for completion of medical reports and duplicate copies of medical records is stated below.

However, we may experience a delay in processing some requests due to the following reasons:

  • Patient has upcoming clinic appointments or has been warded
  • Patient has multiple reports requested from several clinical departments
  • Doctor is away on leave

Notes:

  • Ordinary Medical Report
    Provides the relevant facts about the medical condition and treatment/medication the patient is receiving. Report can be in the form of a written report or completion of medical insurance claims questionnaire.
  • Specialist Medical Report
    Written by specialists in response to requests that require a specific opinion or prognosis from the specialist based on the medical facts of the case.
  • Second Opinion Medical Report
    Expert opinions written by KK specialists on patients not seen at KK hospital.
  • Medical Board Report
    Written by KK medical board to assess KK patient’s suitability for continued employment with an organisation.
  • Simple Insurance Form
    Provides the final diagnosis, name/type of surgical procedure performed, date of admission/discharge from hospital. Form is completed by DDMS staff.

Please note that should you decide to cancel your request, there will be a cancellation charge of SGD$26.75 for ordinary medical report and SGD$53.50 for specialist medical report.
