Attachment programme in Sterile and Non-sterile Compounding

Description

To provide an opportunity for pharmacists to increase their knowledge and expertise in running a pharmacy compounding service.

Duration

Two weeks

Entry Criteria

Practising pharmacist. Local and foreign practising pharmacists may apply.

Expected Learning/Learning Objectives

• Gain exposure to work processes and operational/technical issues that may be encountered in such a service
• Know the principles of aseptic dispensing and its application
• Gain insight into the principles of aseptic technique validation (for participants to set up their own programme for their own facility: we are not able to validate external personnel as validation should be performed in the environment that the personnel will eventually work in. Validation is not transferable)
• Aseptic compounding guidelines (USP 797 and PIC/S Good Manufacturing Practice) will be discussed and participants will be able to see of how we put the guidelines into practice
• Understand good practices in non-sterile extemporaneous compounding
• Safe handling of cytotoxic drugs (e.g the use of closed systems transfer devices) will be discussed
• Aspects of cleanroom maintenance will be shared

Clinical Attachment Programme in Oncology

Description

To provide an opportunity for pharmacists to increase their knowledge and expertise in Paediatric and Gynaecologic Oncology in KKH.

Duration

Five weeks

Entry Criteria

Practising pharmacist

Expected Learning/Learning Objectives

On completion, the trainee will:

• Understand and be able to apply the concept of safe handling and disposal of cytotoxic drugs
• Develop an understanding in common Oncology drugs
• Be able to counsel newly diagnosed cancer patients on managing common side effects of chemotherapy
• Understand the roles & duties of a clinical oncology pharmacist
• Be exposed and equipped with an understanding of the management of common Paediatric malignancies (ALL) and Women malignancies (Breast & Ovarian Cancer)
• Understand the concept of Hematopoietic Stem Cell Transplant (HSCT) in paediatric patients and its related complications

Attachment programme in Paediatric medicines

Description

To provide an opportunity for pharmacy technicians or pharmacists to be exposed to paediatric medicines used in KKH.

Duration

From two to four weeks

Dispensing Pharmacy Technician/Practising Pharmacist

Expected Learning /Learning Objectives

On completion, the trainee will:

• understand the general concepts in paediatric drug prescribing
• exposure to the range of paediatric drugs used in KKH and their criteria for selection/inclusion into hospital formulary
• understand the special considerations when prescribing drugs for paediatric patients
• familiarise with paediatric dosing and calculations
• familiarise with common references used for paediatric dosing
• exposure to preparation of extemporaneous syrups
• exposure to clinical practise in paediatric medicines (for Pharmacists)

Description

To provide an opportunity for pharmacy technicians or pharmacists to be exposed to drug used in O&G and Neonatology, KKH.

On completion, the trainee will:

• understand the general concepts in O&G and Neonatal drug prescribing
• exposure to the range of specialised women drugs used in KKH and their criteria for selection/inclusion into hospital formulary
• understand the special considerations when prescribing drugs for neonatal patients as well as antenatal/lactating women
• familiarise with neonatal dosing and calculations
• familiarise with the common drug references used in pregnancy or lactating mothers as well as neonates
• exposure to preparation of extemporaneous syrups
• exposure to clinical practise in O&G and Neonatology medicines (for pharmacists)

KKH Paediatric Pharmacy Residency Programme

Overview

This 12-month programme is designed to train pharmacists towards specialized practice focused on the care of paediatric patients by providing broad based clinical pharmacy training in a children's hospital / referral centre. The residents will be exposed to a wide range of paediatric specialties and have hands-on experience to care and manage patients’ disease. Through the programme, the resident will develop into an independent and competent paediatric pharmacist with expertise in patient care, teaching and research.

Objectives

This programme adheres to the ASHP International Accreditation Standard for Postgraduate Year Two (PGY2) Paediatric Pharmacy Residency Programs.

The Six Educational Outcomes that the resident will achieve at the end of the residency are as follows:

Outcome R1: Having leadership and practice management skills in the paediatric patient care setting.

Outcome R2: Optimize the care of inpatient and outpatient paediatric patients by providing evidence-based, patient-centred medication therapy as an integral part of an interdisciplinary team.

Outcome R3: Serve as an authoritative resource on the optimal use of medications used to treat paediatric patients.

Outcome R4: Evaluate, manage, and improve the medication-use process in paediatric patient care areas.

Outcome R5: Demonstrate excellence in the provision of training or educational activities for paediatric health care professionals, health care professionals in training, and the public.

Outcome R6: Conduct paediatric pharmacy research.

Course Contents

 
* Each rotation is allowed for up to 1 repeats.
# Residents, with no prior training experience in paediatric setting, will undergo 2 months of rotation in General Paediatrics.
+ Half a month of protected time for finalising project, data analysis and writing report.

Target Audience and Expected Competencies

• Holder of the degree (or higher) in pharmacy that is recognized by the Singapore Pharmacy Council (SPC).
• Registered to practice Pharmacy (patient care) in Singapore. Overseas applicants must be eligible for pharmacist licensure in Singapore and must have attained licensure before the initiation of residency.
• Have at least three years of direct patient care experience in the 5 years immediately preceding the application and a postgraduate degree (MSc or Pharm D)
  • Non postgraduate degree may be accepted on case-by-case basis
• All applicants should demonstrate that they possess sufficiently broad knowledge, skills, attitudes, and abilities in pharmacy practice necessary for further professional development at an advanced level of pharmacy practice. These will be assessed through applicants’ curriculum vitae, qualifications, supporting documents and interview with applicants.

Exit Requirements

• All Residents are required to complete the 12-month programme.
• Meet all Residency requirements including making sufficient progress towards all the required goals and objectives as evidenced by either satisfactory progress or achieved being marked for all goals.
• Satisfactory completion of all learning experiences as evidenced by all required work assigned being completed to the satisfaction of preceptors.
• Completion of residency major project with a manuscript that is approved by the project supervisor and APD/RPD.
• Compliance with institutional and departmental policies.
