Accessing Your Discharge Documents

You can access discharge documents on the Health Hub app without waiting for them to be printed.

The digital medical certificate will also be sent to you via SMS.

If you require follow up at our specialist clinics, the appointment date and time will be sent to you via SMS.

Access Your/Your Child’s Discharge Information on Health Hub in 3 Easy Steps:

Step 1: Download the Health Hub app from App Store or Play Store. Login to the Health Hub app with your SingPass.
