Day of Admission

On the day of your admission, please come at your appointed time with the following original documents:

• Admission Authorisation Form
• NRIC / Birth Certificate (if under 15 years old)
• Completed Medisave Authorisation Form
• Medical Benefit Identification documents

Civil Servants and dependants
Documents Required

• Civil Service Card (CSC)
• Public Service (PS) Card
• Service Injury (SI) Card

Employees of other organisations and dependants
Documents Required

• Hospitalisation Identity Card (HIC)
• Letter of Guarantee (LOG) from employer or insurance company

Foreigners working in Singapore
Documents Required

• CPF membership card
• LTVP Plus and S Pass or work permit
• Employment pass

Foreigners not working in Singapore
Documents Required

• Passport
• Dependent pass or student pass
• Letter of Guarantee (LOG) from insurance company

If you need to change or cancel your appointment, please inform your doctor at least two days in advance. You can reach your doctor through our Call Centre Assistants at tel: +65 6225 5554 (press '0').

If you have already indicated your bed choice, please also inform our Admissions staff at tel: +65 6394 1200.

Our Admissions counters' operating hours are as follows:

• Monday to Friday: 7.00am to 6.00pm (excluding Public Holidays)

If you need to be admitted after 6.00pm, please proceed to:

• Urgent O&G Centre, Basement 1, Women's Tower for women
• Children's Emergency, Basement 1, Children's Tower for children
