FAQ on use of Caregiver Card

Q: Where can I obtain a copy of the Caregiver Card?

A: For a child patient, one Caregiver Card will be issued at the time of registering the child for admission. For women patients, the card will be issued on a needs basis by the ward nurse.

Q: What benefits does the Caregiver Card have for me?

A: The Caregiver Card identifies the bearer as the person assigned to care for the patient during the patient's stay in the hospital. This allows the card bearer to visit and care for the patient outside of visiting hours. Please produce the Caregiver Card at the security counter when visiting after 8.00pm at night.

Q: Can I request for an additional card?

A: An additional Caregiver Card may be issued on a needs basis by the ward nurse.

Q: I have lost my Caregiver Card, what should I do?

A: You may report the loss of your card to the ward nurse and request for a replacement card. Please note that a replacement charge may be imposed.

Q: After the patient is discharged, can I keep the Caregiver Card for future use?

A: No. The card is only valid for the duration of the patient’s current stay. A new Caregiver Card will be issued during a new admission. Please return the caregiver card to the ward nurse upon the patient’s discharge.
