Please inform KKH DPLM at 6394 1354 once confirmed to send specimen after office hour for transport arrangement.
Request form and all specimen containers MUST have CPOE Test label / Patient Info label. KKH logo seal must be signed over with Dr’s signature and MCR no. and placed on both the request form and specimens. Please print labels to paste on all specimens and refer to labelling requirements.
