Mobile Inpatient Care@Home (KKH@Home)

What is KKH@Home

KKH@Home is an alternative inpatient care delivery model that offers clinically-suitable patients the option of being hospitalised in their own homes, instead of a hospital ward.

Patients will be provided with clinical care and home treatment by the care team which comprises of doctors, nurses and allied health professionals.

From 1 April 2024, KKH@Home is recognised and accepted model of care in Public Healthcare Institutions.

How will patients be cared for at home?

  • Combination of teleconsultations and home visits until fit for discharge
  • Home delivery of medications
  • Round-the-clock access to the care team
  • Alerts sent to care team immediately for abnormal vital signs
  • Care escalation protocols in place, should medical condition deteriorates

Eligibility

Patients who:

  • Are clinically stable
  • Are not anticipated to require intensive hospital care
  • Are independent or have adequate caregiving support at home
  • Have telecommunication access (i.e. stable internet connection at home)
