1. How do I apply for a medical report/ record?

For Individuals without SingPass	Steps	Consent Form
Hard-Copy Form Submission (Via Email)	Email the following mandatory documents to "Insurance.GenEnquiry@kkh.com.sg" Mandatory Documents to submit together with the application: - Complete the attached “Consent for Release of Medical Information Form” Insurance Form to be fill by Doctor/ Hospital (if applicable) For Patient 21 years and above Copy of Patient’s NRIC (front & reverse) Copy of valid passport or identity document issued by Singapore authorities (for non-residents) For Patient below 21 years old Copy of Patient’s Birth Certificate Copy of 1 Parent’s NRIC (front & reverse) Copy of valid Passport or Identity Document issued by Singapore authorities (for non-residents) Payment instructions will be sent via email within 3 working days from date of receipt the consent form and all the mandatory documents. # Delays is expected for any incomplete submission. 2. When can I receive my medical report/ record?

However, your request may take longer to complete if: Patient has upcoming / frequent clinic appointments Patient has been admitted to the ward Patient has multiple reports requested from several clinical departments Doctor is away on leave # To support environmental sustainability, all completed medical report will be encrypted and sent out via email. No hard copy will be mailed.

4. How can I obtain the completed medical report in original hard copy?

The default delivery mode for completed medical report is email. No hard copy will be mailed by KKH.
Applicant/ Patient who require the completed medical report in original hard copy, he/ she has to self-arrange the collection from KKH.
KKH Medical Reports Unit will liaise with the applicant/ patient on the collection date & time from KKH Concierge Counter once the requested medical report is ready.
Applicant/ Patient may select either 1 of the collection option :-
- To Collect Personally
  - Upon collection, please bring along your NRIC for verification purpose
- To Authorise a 3rd party to collect on behalf (Example: Family Member/ Insurance Agent/ Legal Firm Representative/ Courier Services Provider)
  - Before collection, applicant/ patient to complete and email the “Authorisation for Collection of Medical Reports” form to KKH Medical Reports Unit.
  - Upon collection, authorised 3rd party to bring along his/ her NRIC and a copy of the signed “Authorisation for Collection of Medical Reports” form for verification purpose.
KKH reserve the right to withhold the release of Medical Report upon collection if the verification process failed.

5. What are the different types of medical report/ record and theirs charges?

2	Completion of Insurance Form By Doctor	It will be completed by a doctor. The insurance form is issued by an Insurance Company. The form is usually in questionnaire format and requires doctor input on details of the medical condition/ treatment the patient received.	S$141.70
3	Specialist Medical Report/ Completion of Insurance Form with Question(s) on Prognosis	This is a comprehensive medical report that is prepared by the patient's specialist in response to requests that require a professional opinion with regards to the patient's prognosis.	S$294.30
4	Ordinary Medical Report (Psychiatric)	This is a factual report of the patient's Psychiatric medical condition.	S$264.90
5	Specialist Medical Report (Psychiatric)	This is a comprehensive medical report that is prepared by the patient's psychiatric specialist in response to requests that require a professional opinion with regards to the patient's prognosis.	S$489.00
6	Doctor's Memo	This is a brief statement from a doctor indicating patient's diagnosis with no explanation of medical condition. Doctor's memo applies to one specific outpatient specialist visit only.	S$21.80
7	Duplication of Medical Certificate	Duplicate copy of issued Medical Certificate.	S$12.00 per copy
8	Duplication of Laboratory Results/ Investigation Results	Selective duplicate copy of lab/ investigation results (Example, Blood test, Investigation reports).	S$12.00 per request
