For first appointment, please bring along:

  1. Identification documents
    • NRIC/Birth Certificate (for children under 15 years old)
    OR
    • Entry/ Re-entry Permit / Employment Pass / Dependent Pass / Work Pass / Student Pass / Visit Pass or any other identity document of your nationality
  2. Referral letter from your family doctor / polyclinic, if any
  3. Your child’s immunisation booklet, for immunisation visits
  4. Any prior medical reports / results, investigation results and images relevant to your medical condition
  5. Baby Bonus / Pioneer Generation (PG) / Community Health Assist Scheme (CHAS) cards (if any)
  6. Medical Benefits Card / Hospitalisation Identity Card (HIC) if your employer has signed an agreement/ letter of guarantee (LOG) with KKH
  7. Letter of guarantee from the National Blood Centre for blood donors and their nominees
  8. Letter of guarantee from your employer/ insurance company certifying that your bill will be settled

If you are currently taking any medication, please bring them along to show the specialist(s) so that they have a better understanding of your health.

For subsequent appointment, please bring along:

  1. Appointment Card
  2. Any form for further investigation(s) ordered, that may have been given to you during your earlier visit.
    E.g. x-ray, ultrasound, mammography, blood test
