Discharge Checklist

Before you go, use this checklist to make sure you're ready for discharge:

  I have all my belongings.

  I feel well enough to go home (no dizziness, nausea, or excessive pain).

  I have received my medications or prescriptions.

  I have been told how to care for my wound or surgical site.

  I know what symptoms to watch out for and when to seek medical help.

  I have a follow-up appointment date (if needed).

  I have received a medical certificate or hospitalisation leave (if applicable).

  I know how to get help if I have any concerns.
